비난 없이 timeline, impact, detection, contributing factors, 잘된 점, 어려운 점, action item을 쓴다. “사람이 실수”에서 멈추지 않고 왜 시스템이 허용·확산·늦게 감지했는지 본다.

action은 owner, due date, verification을 갖는다. “주의한다” 대신 constraint, canary, alert, test, permission, restore drill을 만든다. 동일 계열 사건이 감소했는지 추적한다.
